Survey Monkey is a forms-creation tool that some clinicians use for intake forms and assessments. They offer a Business Associate Agreement, so HIPAA compliant use is possible. Their explanation of their own internal security and their suggestions (click here) to help you keep your clients’ information safe is written in plain language. It reiterates the point that HIPAA compliance isn’t just a “sign the BAA and you’re done” kind of deal.
